Case of the Month

By Dr Ho Chee Keong, MBBS (Mal), M.Med (Radiology) [UKM], AM (Mal)

Answer to Case Of the Month

The diagnosis is adenomyomatosis of the gallbladder.

Adenomyomatosis
The Oral Cholecystogram showed Rokitansky-Aschoff sinuses, which is pathognomonic of adenomyomatosis.

Adenomyomatosis is a form of hyperplastic cholecystoses. It is characterised by mucosal hyperplasia and thickening of the muscular layer of the gallbladder. Herniations of the mucosa into muscular layer are called Rokitansky-Aschoff sinuses. Deposition of cholesterol crystals in these sinuses give rise to the comet-tail artifacts seen on Ultrasound. Adenomyomatosis occurs equally in both men and women.
