The 2010 College of Radiology Value Added Mammogram “Mid-Year” Report
Compiled by Dr Evelyn Ho

Summary of the CMP 2001-July 2010

As of 31 July 2010, the CMP which has been running from 2001 to 2010 has benefitted 6,764 women all over Malaysia. In the past few years, the CMP has concentrated in serving the East Coast and Johor where mammogram services are not so well developed and where the people need more breast health awareness. The CMP is a holistic programme that includes psychosocial support through our partners, the breast cancer support groups in the respective towns/cities where the programme is made available.

CMP 2010 as of 31 July 2010
A total of 477 women have accessed the College of Radiology Value Added Mammogram Programme (CMP 2010) as of end July 2010. 61% were from Johor, 38.8% from Pahang and 0.2% from Terengganu. A total of 7 histology proven breast cancers were discovered amongst 4 Malay and 3 Chinese women. The ages of the ladies with breast cancer ranged from 34 to 54 years. 64.4% had their mammograms for the first time.

The COR Mammogram Programme in IIUM Breast Centre, Kulliyyah of Medicine, Kuantan, Pahang was held between 9 December 2009 until 4 March 2010. The total number of women who had mammography in this programme was 200.

There were 86 Chinese women, 109 Malays and 5 Indian participants. They were from Johor, Pahang and 1 from Terengganu. The women were aged between 34 and 71 years. Most of them (128 women) never had a mammogram before. Among the 200 patients, 22 had a family history of breast cancer. 70 women had breast ultrasound as a complementary examination.

98 women had normal mammograms, 84 had benign findings, 9 had probably benign findings, 6 had slightly suspicious findings and 3 had highly suspicious findings on their mammograms. Seven women had biopsies; and 2 had fine needle aspiration, (1 of a breast lump and 1 of an axillary lymph node). The total number of proven cancers was 5 (mucinous adenocarcinoma – 1, invasive cancer and reactive lymphoid hyperplasia -1, invasive ductal cancer -3)

Participants and providers in Puteri Specialist Hospital, Johor Bahru at the soft launch of the CMP2010 in Johor Bahru

There were 277 participants accessing the programme in Johor Bahru from June to July 2010. The majority of these were Chinese (268) whilst there were only 9 Malay participants. The age range were from 38-73 years whilst the average age was 52 years. 179 had their mammogram for the first time during the CMP2010. The majority of the participants had normal mammograms (233), whilst 35 had benign findings, 5 had probably benign findings and 4 had slightly suspicious findings. Of the latter, two had proven cancer with mastectomy done/to be done whilst 2 were found to have benign/probably benign findings for follow up.

The Breast Cancer Support Group, Johor Bahru is the portal for access to the CMP2010 in Johor Bahru and have been working well with the CoR accredited mammogram provider Puteri Specialist Hospital to ensure smooth flow of the programme. All participants were given their results after the mammogram and the results were explained to them by the consultant radiologist. This is a prerequisite of the CMP, although it may not be practical sometimes when the provider is very busy. The appointment system was encouraged to ensure the mammogram provider is not swamped with too many participants at one time resulting in lack of time to speak/counsel them.

Puteri Specialist Hospital (PSH) now gives the report and images in a CD, instead of films unless the mammograms are abnormal and need further investigation/treatment in the centre of the participants’ choice. PSH is now using a picture archiving and storage system and the imaging department has gone digital.

Once again, very special thanks to the CoR’s partners in breast health, the Breast Cancer Support Society, Segamat, IIUM Breast Centre, Breast Cancer Support Group Johor Bahru and Puteri Specialist Hospital for enabling the programme via electronic means for the CoR. This has helped to save money on administration of the programme. Naturally the cost of mammograms and biopsies would not be possible without the Estee Lauder Companies Malaysia’s generous donation for the 2010 CMP
