Terms and Conditions

  1. This contest is open to all residents of Malaysia (18 years old and above) except Members of the College of Radiology, sponsors, suppliers and their immediate families.

  2. The Radiology Malaysia-Meditel 2005 MySTORY: CeritaKu contest begins on the 1 September 2005 and ends 31 December 2005.

  3. My Story or CeritaKu will be the Main Title of the composition. This may be written in either Bahasa Malaysia or English. You may add a subtitle to the main title.

  4. Length of Your Story: Not more than 1000 words

  5. Language: English or Bahasa Melayu

  6. Submit in Microsoft WORD document format, Times New Roman Font Size 12, Double Line Spacing, A4 page setup.

  7. All entries must be accompanied by full details as shown in the section on “Requirements & How To Submit Your Entry”.

  8. Multiple entries are allowed but each person is only entitled to win one (1) prize (or 1 prize set).

  9. Late entries will be disqualified automatically.

  10. Entries may be submitted by e-mail or by post. Entries by post must be printed on A4 paper, using only 1 side of the paper and the pages must be numbered.

  11. Entries submitted electronically must be received by midnight 31 December 2005. If submitted by post, it should be received by 31 December 2005. Proof of posting does not confirm receipt of the entry.

  12. Prizes will be awarded tentatively by the first quarter of 2006. Winners will be notified of the exact date and venue of the prize presentation.

  13. Prizes are not exchangeable for cash. The College of Radiology reserves the right to substitute the prizes with others of equal or greater value without prior notice.

  14. Prize winners must assume all liability for any injury, damage or claim caused by participating in the contest or use or redemption of any prizes. The College of Radiology and its sponsors will not be held responsible for any such liability.

  15. Judges’ decisions are final. No appeals will be entertained.

  16. All winners will be notified via email (preferred) or by telephone. Please ensure that your entries include a valid email address, correspondence address and contact telephone number. A list of prize winners will be published at www.radiologymalaysia.org.

  17. Winners will be required to attend a prize presentation and/or participate in publicity programmes. Failure to do so may result in the prize being forfeited and no further claims will be entertained.

  18. The College of Radiology reserves the right to use the photographs of the prize winners as well as the submitted stories in post event articles/reports, to print, display and also to publish on the website all winning entries without prior consent or compensation.

  19. The entries will be Copyright College of Radiology.

  20. The College of Radiology reserves the right to amend the above Terms and Conditions at any time without prior notice.

  21. By participating in this contest, participants agree to be bound by the terms and conditions and decisions of the College of Radiology or its agency.

Syarat & Kenyataan Peraduan “MyStory: CeritaKu” Radiology Malaysia-Meditel 2005

 
  1. Peraduan ini terbuka kepada semua yang bermastautin di Malaysia dan berumur 18 tahun ke atas. Ahli-ahli Kolej Radiologi, kakitangan penaja, agensi penaja serta ahli keluarga terdekat (ibubapa, pasangan dan anak) tidak layak menyertai kuiz ini.

  2. Peraduan ini bermula pada 1 September 2005 dan berakhir pada 31 December 2005.

  3. Karyaan anda akan bertajuk “CeritaKu”.

  4. Panjang cerita anda tidak boleh melebihi 1000 patah perkataan.

  5. Anda boleh menulis “CeritaKu” dalam Bahasa Malaysia ataupun Bahasa Inggeris.

  6. Hantarkan cerita anda dalam bentuk Microsoft WORD, Font (bentuk perkataan) Times New Roman saiz 12, Double Line Spacing, setup mukasurat - saiz A4

  7. Setiap penyertaan mesti dilengkapi dengan butir-butir anda seperti tertera di seksyen “Bagaimana menghantar cerita anda”.

  8. Anda boleh menyertai peraduan ini lebih daripada sekali tetapi hanya boleh memenangi satu (1) hadiah (ataupun 1 set hadiah) sahaja.

  9. Penyertaan yang tidak lengkap ataupun terlewat daripada 31 December 2005 tidak akan diterima.

  10. Anda boleh e-mel cerita anda (cara terbaik) atau mengeposnya kepada kami. Cerita yang dihantar melalui pos mesti dicetak pada kertas A4, diguna pada sebelah sahaja dan dinomborkan.

  11. Cerita yang dihantar secara elektronik mesti diterima sebelum 12 malam, 31 December 2005. Jika penyertaan adalah melalui pos, ia mesti diterima sebelum ataupun pada 31 December 2005. Bukti mengepos bukan bukti penerimaan.

  12. Hadiah-hadiah pemenang mungkin akan diberi menjelang akhir Mac 2006. Pemenang-pemenang akan diberitahu tarikh dan tempat penyampaian hadiah.

  13. Hadiah-hadiah tidak boleh ditukar untuk wang tunai sama ada sebahagian ataupun sepenuhnya. Sekiranya Kolej Radiologi gagal menyediakan hadiah-hadiah yang dinyatakan, Kolej Radiologi berhak menggantikan hadiah tersebut dengan hadiah yang sama nilainya tanpa memberikan notis terlebih dahulu kepada pemenang

  14. Pemenang mestilah menanggung semua liabiliti termasuk kecederaan, kerosakan ataupun tuntutan yang berasal ataupun akibat penyertaan peraduan ini, penggunaan ataupun penerimaan sebarang hadiah peraduan ini. Kolej Radiologi ataupun penaja peraduan ini tidak bertanggungjawab untuk sebarang liabiliti tersebut.

  15. Keputusan Kolej Radiologi dan hakim-hakim adalah muktamad. Sebarang rayuan dan/atau surat-menyurat selanjutnya berkaitan rayuan tersebut tidak akan dilayan.

  16. Semua pemenang akan dihubungi melalui e-mel ataupun melalui talipon. Sila tentukan setiap penyertaan mempunyai alamat e-mel, alamat surat-menyurat dan nombor talipon yang betul. Senarai pemenang-pemenang peraduan ini akan terpapar di laman web www.radiologymalaysia.org

  17. Pemenang diwajibkan untuk menghadiri upacara penyampaian hadiah ataupun sebarang program untuk publisiti laman web/kempen kesedaran Kolej Radiologi. Kegagalan pemenang menghadiri upacara penyampaian hadiah mungkin akan mengakibatkan hadiah tersebut ditarikbalik/batalkan. Rayuan selanjutnya tidak akan dilayan.

  18. Kolej Radiologi berhak menggunakan nama, gambar yang diambil atau maklumat lain tentang pemenang serta ceritanya untuk tujuan pengiklanan dan promosi masa depan tanpa kebenaran pemenang dan tanpa bayaran kepada pemenang. CeritaKu yang diterima juga mungkin dipaparkan di laman web.

  19. Semua cerita yang diterima akan jadi Hakcipta Kolej Radiologi

  20. Kolej Radiologi berhak pada bila-bila masa tanpa sebarang notis, menambah, meminda, mengubahsuai, menukar atau mengubah syarat-syarat dan kenyataan Peraduan CeritaKu 2005 seperti yang ternyata di sini, secara sepenuhnya atau sebahagiannya, atas budi bicara mutlaknya.

  21. Semua peserta terikat kepada syarat dan kenyataan peraduan yang terkandung disini.

  22. Sekiranya berlaku percanggahan atau pertikaian dalam pentafsiran mana-mana syarat dan kenyataan di dalam ini, sama ada dalam Bahasa Inggeris atau Bahasa Melayu, versi Bahasa Inggeris akan dikuatkuasakan.

Daripada seorang pengidap kanser
payudara

“Diagnosis kanser payudara pada 1998 menjadikan saya berasa hampa dan terpencil yang seterusnya beransur dengan perasaan cemas.... kesakitan kanser payudara bukan fizikal tetapi lebih melibatkan emosi.” Ranjit Kaur, pesakit kanser payudara.

Ranjit sekarang adalah Presiden Persatuan Kebajikan Kanser Payudara di Malaysia serta Presiden bagi Jaringan
‘Reach to Recovery International Breast Cancer Support Network’.

Daripada seorang suami yang isterinya baru didiagnosis kanser...
“Bagi kali pertama dalam kehidupan saya, saya menangis teresak-esak apabila isteri saya telah didiagnosis dengan kanser payudara pada umur 39 tahun.”

Daripada seorang adik perempuan ...
“Saya ingin berkongsi dengan anda semua kepentingan pengetahuan dan kesedaran – dan betapa teruknya kejahilan mengenai kanser di kalangan sesetengah orang. Walaupun kematian ibu dan kakak saya menyebabkan saya menderita, peristiwa itu juga sebuah ‘hadiah istimewa’ bagi diri saya…. Malangnya saya terus jahil mengenai kanser bagi masa bertahun-tahun lagi. Saya tidak tahu bahawa tumor otak ibu saya adalah kanser sekunder manakala kanser payudara menjadi kanser primer sehingga 13 tahun kemudian!”

Tricia Cronin, adik dan anak kepada dua wanita yang menghidapi dan kemudian meninggal akibat kanser payudara. Adegan ini adalah dari pada cerita Tricia semasa dipersembahkan di Persidangan Pertama Reach to Recovery International Breast Cancer Support bagi Asia Pasifik pada Ogos 2002.
 




Daripada seorang pengidap kanser muda yang telah mencuba rawatan alternatif...

“Semasa diagnosis kanser payudara saya, seorang kawan telah memperkenalkan saya kepada rawatan alternatif. Seorang ‘pakar’ herba dari Pulau Pinang melihat rekod perubatan saya dan berkata bahawa herba ini akan ‘mengeluarkan’ kanser saya ini. Mereka mengujarkan saya supaya tidak menjalankan pembedahan. Herba itu berharga RM700-RM800 sebulan dan mereka hanya menerima wang tunai….. rawatan itu sungguh leceh. mahal dan tidak berkesan. Saya berhenti mengambil herba itu. Saya kemudian menjalankan surgeri saya serta melalui kemoterapi dan mendapatinya tidak menakutkan seperti yang disangka. Saya dapat menahan rawatan ini dan rambut saya tumbuh semula!”

Mei Quin,pesakit kanser payudara dan sukarelawan Persatuan Kebajikan Kanser Payudara. Mei Quin dalam lingkungan umur 30an apabila didiagnosis dengan kanser. Beliau juga telah dicalonkan bagi Anugerah Humanis Muda NST-Price Waterhouse pada tahun 2004.

“Saya berjaga-jaga supaya tidak memberitahu keluarga terlalu banyak maklumat kerana saya tidak ingin keluarga saya melayan saya seperti “ Saya mungkin akan mati esok” ! Saya juga menasihati kawan saya supaya tidak melayani kaum kerabat mereka yang menghidap kanser seakan-akan mereka tidak ada harapan bagi masa depan mereka. Mereka yang dihidapi kanser perlu dilayani seperti biasa sebanyak boleh.”

From a mother with advanced cancer…

“I am careful not to say too much to my family as I do not want them to treat me like “I might just die tomorrow”! I also counsel my friends never to treat their relatives with cancer as if they have no more hope. Those with cancer should be treated as normally as possible.”

Lessons from a caregiver

“Unlike presents, your presence is less straight-forward. To be frank, Ewe Jin and I found some people’s presence uplifting and others’ strength-sapping. Before you visit someone with cancer, ask yourself the purpose of your visit. Don’t go if it is just to reassure yourself that your loved one is all right. In all likelihood, the person may be fighting the disease well but side-effects of therapy are beginning to show – perhaps mouth blisters, scorched skin, a sorethroat and so on. In such a condition, having to reassure visitors is no easy task and really unfair on the patient!”

From a cancer survivor

“A diagnosis of breast cancer in 1998 left me with feelings of despair and isolation, alternating with feelings of panic……… The pain of breast cancer is not physical, but emotional.”

Ranjit Kaur, Breast Cancer Survivor. Ranjit is now President of the Breast Cancer Welfare Association in Malaysia and also the President of Reach to Recovery International Breast Cancer Support Network.
 

From a young cancer survivor who tried alternative therapy…

“At the time of my diagnosis with breast cancer, a friend introduced me to alternative treatment. A so-called “expert herbalist” from Penang saw my medical records, and said the herbs would remove the disease at its roots, and not just superficially. They told me not to go for surgery. The herbs cost me about RM700-RM800 a month, and they would only accept cash….. It was so troublesome, expensive and ineffective. I have stopped taking the herbs. I went for my surgery and then, chemotherapy and it is not as scary as one thinks. It is tolerable and my hair grew back!”

Mei Quin, Breast Cancer Survivor and volunteer with Breast Cancer Welfare Association. Mei Quin was in her 30s when cancer was diagnosed. She was nominated for the NST-Price Waterhouse Young Humanitarian Award in 2004.
 


From a sister who remained ignorant for 13 years…

“What I want to share with you is the importance of knowledge and awareness – and just how incredible ignorance can be. Although the death of my mother and sister has caused me grief, it has also been a wonderful gift of life to me…. Unfortunately, I remained extremely ignorant of cancer for many years to come. I did not find out that the brain tumour my mother had was secondary cancer and that the primary cancer was breast cancer until more than 13 years later!”
